Explanation of above estimates (including long-range effect):
Please explain increases and decreases in personal services, current expenses, repairs and alterations, assets, other costs and revenues, including assumptions and data sources and delineation between start-up and ongoing costs. Please also include a long-range schedule of costs and revenues if fiscal impact is expected to vary in future years.
Estimates are based on the 100% of 4,500 MDs with a primary practice locations in WV. therefore 5,000 decals manufactured, estimated cost $1,2000.00
Database development of an on-line application 20 hours estimate at $130.00 per hour, estimated cost $2,600.00
Staff time to process and issue decal would be approximately 1/2 hour per application request. based on 20 dollars per hour the total cost would be approximately $50,000.00 processing cost.
To mail decals visa certified mail, 5000 decals at $4.40 per issuance is $22,000.00
Estimated administrative penalties are unknown.
